Medicare Rebates available below, you will need to get a referral from your GP.

-Chronic Disease Management Plan (CDM) Individual Appointments (up to 5 per year) - Rebate of $61.80

-Allied Health Services for Aboriginal and Torres Strait Islander People (up to 10 per year) - Rebate of $61.80

-T2DM Initial Assessment - Rebate of $79.30

-T2DM Group Class (up to 10 per year) - Rebate of $19.75 per class (coming soon)
